MAGNOLIA BOXING CLUB, LLC

RELEASE OF LIABILITY

I represent and warrant that I am physically fit to participate in boxing activities at Magnolia Boxing Club, LLC and I have no medical condition that would prevent me from safely engaging in such activities. I agree to inform Magnolia Boxing Club, LLC of any changes to my health status that may affect my ability to participate in activities at Magnolia Boxing Club, LLC. I agree to abide by all safety guidelines and instructions provided by Magnolia Boxing Club, LLC.

I hereby agree to assume all risk attended upon me while participating in events and activities at Magnolia Boxing Club, LLC.

I specifically waive, release and discharge Magnolia Boxing Club, LLC or any of its agents, employees, servants or members from any and all claims for death, personal injury, or property damage which I may have, or which may hereafter accrue to me as a result of my participation at Magnolia Boxing Club, LLC. I agree to indemnify and hold harmless from liability Magnolia Boxing Club, LLC and its members, agents, servants, volunteers or employees by reason of any acts of accidental death, accidental injury or damage to person or property which I may suffer, while participating in any session at Magnolia Boxing Club, LLC.

This Release is intended to discharge Magnolia Boxing Club, LLC and its members, managers, agents, servants or employees by any reason of any accident, death, injury or damages to person or property which I may suffer from and against any and all liability arising out of, or in any way connected with my participation in any event organized at or by Magnolia Boxing Club, LLC, even though that liability may arise out of negligence or carelessness on the part of any individual member, manager, agent, servant or employee of Magnolia Boxing Club, LLC.

I fully understand and agree that this waiver, release and assumption of risk is binding upon me, my heirs and assigns and I agree to assume all responsibility.
